For a copy of your medical records, contact Dr. Ceponis’ office at 613-584-3398. Leave one message with your name and date of birth. You will be contacted once your chart has been processed and available for pick up. Record requests will be processed on a non-urgent basis and incur a fee as record transfer is not covered by OHIP
